Provider Code of Conduct
Our custom wellness program allows you the opportunity to meet with a highly credentialed provider to develop a wellness program specifically designed to meet your needs and interests. This wellness professional will assist you by teaching, guiding, and inspiring you to achieve your goals.
Personal wellness is based on finding the correct fit to ensure a comfortable client-provider relationship. If the relationship is not beneficial to both parties, the Neighborhood Studios' staff reserves the right to assign another provider to the client or services will be discontinued. No refunds will be processed.
• The Client-Provider relationship is based on ethics and is built on confidentiality, trust, and honesty.
• Physical contact for proper form and adjustment is permissible upon client's consent; communicate your level of comfort with your provider if you feel otherwise.
• The professional should not discriminate against clients based on race, color, national origin, religion, or on any other basis that would constitute illegal discrimination. This being said, providers should know their professional limitations and not engage in prescriptions for clients with special needs that the provider is not educated or trained to manage safely or effectively.
Wellness Professional Policies
____ I understand that appropriate communication occurs via email unless otherwise noted by the provider. Communication should only pertain to the professional client-provider relationship. Clients may contact the Neighborhood Studios' staff at 703-791-9480 as a secondary form of communication if needed.
____ Sessions must take place at the location designated in the contract.
____ I acknowledge that appointment times are reserved and that cancellations must be made by 5:00 pm the previous business day for all appointments.
____ I understand that I will not receive a refund for missed appointments. If a cancellation is not made by 5:00pm on the previous business day, the client understands that he/she will be charged for the session. Likewise, a ‘no show’ counts as a session serviced and the client will be charged.
____ I understand that appointments will begin and end promptly as scheduled. Any delays to the start of a scheduled appointment will not be a cause to extend the provided service beyond the remainder of the scheduled time; I will not expect or ask my provider to run overtime. I understand that if I am 10-minutes late, my session will be cancelled, and I will be charged for that session.
____ I am fully aware of the risks associated with participation in physical activity and voluntarily agree to participate in the exercises recommended by the trainer. I agree to fully comply with the code of conduct and client policies provided by the staff at Neighborhood Studios. Further, I understand that services will be discontinued should I fail to comply with the policies stated.
